Beginning last week, April 20, certified medical examiners must use versions of the official Medical Long Form and Medical Examination Certification (MCSA-5876; i.e. med card) approved by the Office of Management and Budget when conducting physical exams of commercial motor vehicle drivers. Previously, Medical Examiners were permitted to use forms that were "substantially similar" to those shown in the Federal safety regulations. The new forms reflect changes required under FMCSA's 2015 Medical Examiner's Certification Integration Final Rule and cannot be materially altered. The OMB approved forms can be found at the National Registry of Certified Medical Examiners website along with a list of Frequently Asked Questions on their use.
